Job summary

Kentucky River Community Care is seeking a Community Support Associate to provide community-based support services in Letcher County Outpatient, Schools, and IOP settings. This regular full-time position supports individuals to live with maximum independence in their communities.

The role requires High School/GED and at least 1 year of full-time experience working with individuals receiving services for mental health treatment or co-occurring disorders.

Qualifications

• Must have at least 1 year of full-time experience working with individuals who receive services for treatment of a Mental Health Disorder and/or co-occurring disorder.
• Meet the requirements for a department-led or approved training program within six months of hire.
• Must complete within six months the department-led or approved training program.

Responsibilities

• Conduct activities enabling individuals to live with maximum independence in community-integrated housing.
• Ensure successful community living via skill training, cueing, or supervision per the treatment plan.
• Remind recipients to take medications and monitor symptoms and medication side effects; teach parenting, budgeting, and community resource access.
• Meet requirements for comprehensive community support services established in 908 KAR 2:250.

MUST COMPLETE WITHIN SIX (6) MONTHS OF HIRE THE DEPARTMENT-LED OR APPROVED TRAINING PROGRAM.

Primary Functions:

• Conduct activities necessary to allow an individual to live with maximum independence in community-integrated housing;
• Ensure successful community living through the utilization of skill training, cueing, or supervision as identified in the recipient's treatment plan;
• Include: reminding a recipient to take medications and monitoring symptoms and side effects of medications; teaching parenting skills, teaching community resource access and utilization, teaching emotional regulation skills, teaching coping skills, teaching how to shop, teaching about transportation, teaching financial management, or developing and enhancing interpersonal skills;
• Meet the requirements for comprehensive community support services established in 908 KAR 2:250.

Shift/Days & Work Hours: Monday - Friday 8:00am - 4:30pm
