Qualifications

• Requires graduation from an accredited college or university with a major in Social Work (or related field).
• Must have or obtain State of Wisconsin Social Worker certification; temporary or training certificate acceptable initially.
• Valid driver's license and access to reliable transportation for city and rural travel.
• Strong preference for at least one year of experience with the client population served by the unit.

Responsibilities

• Conduct thorough assessments through interviews, home visits and collateral investigations to determine needed services.
• Provide social services per statutes and program guidelines; refer clients to community resources when appropriate.
• Develop and update service plans; participate in care conferences and document progress.
• Input data in computer systems and use email, internet, and intranet to coordinate care.
