Job summary

Qualifications

• Associates Degree in Social Work, or related field, plus two years related experience preferred.

Responsibilities

• Conduct comprehensive client intake and assessments to determine eligibility for services.
• Provide referrals to clients for appropriate services (HSI Infrastructure).
• Provide monthly follow-ups with quarterly re-assessments on each client.
• Maintain Process Notes and filing system for clients/databases.
• Develop and present training and educational programs/workshops.
• Collaborate with other agencies and serve on committees as needed.
