Job summary

UCP Heartland in Saint Louis, Missouri, is seeking a Community Registered Nurse to provide compassionate care for children and adults with differing abilities. This full-time salaried role includes overseeing a caseload, coordinating physician orders, and ensuring accurate medical records.

You will maintain EMAR and HRST data, participate in on-call rotations, train staff, and liaise with medical providers to support client health goals.

Qualifications

• Registered Nurse with active Missouri license.
• One year of RN experience preferred.
• Strong medical assessment and communication skills.

Responsibilities

• Ensure physician orders are current and signed before expiration.
• Review medical records and respond to issues promptly.
• Maintain EMAR per physician orders and coach staff.
• Respond to calls/emails; participate in RN on-call rotation.
• Complete monthly face-to-face client assessments as needed.
• Train and delegate to direct support staff; monitor competency.
• Mentor staff through Level 1 Med Aide training.
• Liaise with medical providers to ensure appropriate client care.
• Attend ISP and transition meetings for care planning.
• Complete required documentation by deadlines.
• Update HRST system and generate Health Risk Support Plans.
• Other duties as assigned.
