Chariton Valley Association DBA Center for Human Services seeks a Community Registered Nurse Coordinator in Adair County. The RN coordinates medical services, medications, health assessments, and transitions, ensuring person-centered care and strong documentation in line with Medicaid, DMH, and HIPAA requirements.

You will supervise Community RNs, provide coaching, participate in on-call rotations, and promote a collaborative, culturally competent care environment while advancing self-care and

Formación

• ADN required; BSN preferred.
• Current RN license to practice in the state.
• CPR certification required; DHSS Instructor cert preferred.
• Must have and maintain CPR and instructor certifications.
• Valid driver’s license and reliable vehicle.

Responsabilidades

• Provide clinical leadership and oversee nursing staff.
• Coordinate medical services, medication management, and health assessments.
• Ensure regulatory compliance and complete documentation per Medicaid, DMH, HIPAA and related standards.
• Train and coach nurses; supervise on-call coverage and care planning.
• Maintain ISPs health sections and communicate with providers.
