Qualifications

Responsibilities

• Connect participants to community resources and services.
• Maintain accurate case documentation and ensure compliance.
• Provide advocacy, crisis support, and skill-building activities.
• Coordinate services with staff and community partners.
• Monitor participant progress and update plans as needed.
