Job summary

Across The Life Span, PLLC is seeking a Qualified Professional to deliver community-based behavioral health services under NC Medicaid CST and ICS definitions. The role focuses on individualized support, skills training, and care coordination to promote recovery and community integration.

The position serves consumers across Yanceyville, Roxboro, and Greensboro with collaboration across an ATLS multidisciplinary team to ensure documentation compliance and achievement of treatment goals.

Qualifications

• Bachelor's degree in a human services field and 2+ years in the target population
• Bachelor's degree in any field and 4+ years in the target population
• Associate's degree in a human services field and 4+ years in the target population
• High school + 6+ years in the target population

Responsibilities

• Provide individualized community support services including skills training in daily living, social functioning, illness self-management, and vocational readiness
• Deliver psychoeducation to consumers and supports regarding mental health symptoms, medications, coping strategies, and recovery principles
• Conduct functional assessments and contribute to PCPs and Individualized Care Plans
• Provide crisis support planning and safety plan reviews with clinical supervision
• Assist consumers with accessing community resources including housing, transportation, benefits, and social supports
• Support consumers in developing self-advocacy and engagement in their recovery process
• Accompany consumers to appointments, medical visits, and daily living activities as clinically indicated
• Participate in multidisciplinary treatment team meetings and case conferences
• Coordinate with primary care and psychiatric staff for integrated care
• Collaborate with CPSS peer support team to reinforce milestones
• Communicate status updates and treatment plan changes to Clinical Services Manager
• Facilitate warm handoffs and referrals to higher levels of care as appropriate
• Complete documentation in EHR within established timeframes
• Ensure CCAs, PCPs, and service authorizations are completed accurately and timely
• Maintain documentation in HIPAA-compliant manner
• Participate in internal QA/QI
• Adhere to two-level document review workflow
• Track units of service and complete billing documentation
