The role focuses on coordinating community resources, building partnerships with health and social service providers, and promoting Family Connects Maryland through outreach, enrollment, and scheduling home visits.

Qualifications

• Bachelor’s degree in Social Work, Human Services, or Public Health.
• Six years of progressive experience in early childhood/prevention services.
• Knowledge of infant/child development and maternal health.
• Experience in community-based programs and diverse populations.
• Willingness to travel to multiple program sites.

Responsibilities

• Nurture relationships with community resources that support families.
• Identify resources, establish provider relationships, and fill gaps.
• Provide case consultation to support nurse home-visitors.
• Market the program to referral sources and community groups.
• Increase options for families to learn about home visits.
• Establish a community advisory board for program coordination.
• Assist with initial contact with families to engage and schedule visits.
• Organize enrollment scheduling at Sinai Hospital.
