Qualifications

Responsibilities

• Provide counseling, behavioral interventions, restorative services, crisis management services.
• Monitor and assist residents in household maintenance activities.
• Provide training and assistance to residents in daily activities.
• Assist residents in developing meaningful service plan goals.
• Conduct fire safety training activities.
• Document interactions with residents as per agency standards.
• Meet with primary residents for counseling and service plan goal reviews.
• Assist residents in maintaining financial entitlements.
• Monitor medications as required.

The CEC is designed to enhance and promote the integration of medical and behavioral health disciplines to improve patients’ outcomes.

Responsibilities

• Provide counseling, behavioral interventions, restorative services, assistance in negotiating community activities and crisis management services.
• Monitor, train and assist residents in all programs in household maintenance activities to ensure a clean, safe and homelike environment. Directly participate in household maintenance activities when required.
• Provide training, supervision and assistance to residents in daily activities including food purchases, recreational activities, meal planning and personal care activities.
• Assist residents in developing meaningful service plan goals and provide supports to assist residents in achieving their goals.
• Conduct fire safety training activities.
• Document interactions with residents in accordance with agency policies and standards.
• Meet weekly, or as scheduled, with assigned primary residents for counseling and to establish and review service plan goals.
• Assist residents in procuring and maintaining financial entitlements.
• Monitor medications as required.
