Job summary

Charter Healthcare is seeking a Community Relations Nurse Liaison to promote hospice services within the assigned territory and educate facilities, patients, families, and referral sources about care plans. You will collaborate with the Admission Team to ensure timely admissions and accurate documentation.

The role emphasizes field presence, timely patient status reporting, and coordination with care teams to align marketing efforts with local providers and community needs.

Qualifications

• Bachelor's degree in health care related field preferred.
• 2+ years hospice, palliative, home health, or similar experience preferred.
• Strong verbal and written communication skills; must respond to needs of providers, discharge planners, staff, patients and families.
• Proficient in providing education and timely reports to the patient’s care team, facility staff and family as appropriate.
• Valid Driver’s License and current automobile insurance.

Responsibilities

• Assist Admissions with obtaining admission documents and patient financial information.
• Goes into the field and establishes presence of the organization with service members.
• Responds to referrals for evaluation and/or admission.
• Explains the program to patients, family members and other referral sources.
• Works with the Admission Team for referrals, admissions and care coordination.
• Identifies key referral sources and assists in developing a marketing plan for the program.
• Attends in-services, staff meetings and case conferences.
• Completes all documentation accurately and legibly.
• Complies with policies, laws, regulations and standards of practice.
• All other tasks and duties deemed necessary.

POSITION SUMMARY:

The Community Relations Nurse Liaison, in conjunction with the other members of the Admission Team, is responsible for activities and actions related to promoting the services and expertise of the organization to current and potential referral sources, patients and their family members, and the community at large. The Community Nurse Liaison will provide all necessary education to the patient, their family members or caregivers, and facility staff regarding the patient’s progress toward the goals outlined in their plan of care and other areas deemed necessary to provide the highest quality care to the patient.

Director of Operations or Executive Director

QUALIFICATIONS:

Education: Bachelors’ degree in health care related field is preferred.

Credentials: None

Experience: 2+ years of hospice, palliative, home health, or similar experience is preferred.

Core Competencies: Working knowledge of applicable Medicare, Medicaid, and Private Insurance regulations and guidelines. Excellent verbal and written communication skills; must be able to respond to the needs of ordering providers, discharge planners, facility staff, patients, and patient family members. Must be proficient in making meaningful observations regarding changes in patient status and provide accurate, comprehensive, and timely reports to the patient’s care team, facility staff, and the patient’s family, as appropriate. Demonstrated experience in establishing and implementing effective Marketing strategies strongly preferred.

• Works with admission team for referrals, admissions, and care coordination of patient and family needs.
• Identifies key referral sources and assists the admission counselors to develop a marketing plan for the program.
• Attends and participates in in-services, staff meetings and case conferences.
• Completes all documentation in an accurate and legible manner.
• Complies with company policies and procedures, laws and regulations and standards of practice.
• All other tasks and duties deemed necessary and appropriate.
