Job summary

Triangle, Inc. is seeking a Community Coordinator in Malden, MA to support participants through community-based activities and advocacy. This role manages groups, develops opportunities, and coordinates transportation and data collection for ISP-related plans.

The position requires a high school diploma, 1–3 years related experience, and a MA driver's license. Work hours are full-time, Monday–Friday, 8am–4:30pm, at Broadway Day Services.

Qualifications

• High school diploma or equivalent required.
• Some college coursework and/or 1–3 years related experience preferred.
• Massachusetts driver's license and clean driving record required.

Responsibilities

• Supervise groups of participants in work sites, volunteer sites, recreational outings and other community activities.
• Develop and maintain diverse volunteer, vocational, and educational opportunities for individuals.
• Assist with development of new and engaging curriculum to meet program goals.
• Implement independent living, employment, self-advocacy, and soft skills into daily activities and curriculum.
• Transport individuals to and from community activities and employment sites.
• Prepare or assist in the preparation of required assessments and reports (incident reports, safety assessments, progress summaries).
• Participate in identified training and professional development opportunities.
• Intervene in unsafe interactions or behavioral incidents to ensure participants' safety.
• Attend ISP meetings with DDS, participants and families as needed.
• Take directions from Program Coordinator to collect accurate data for ISP's, behavior plans, monthly progress notes, etc.
• Collaborate with other Community Coordinators and Program Coordinator to ensure quality service delivery; report progress and issues regularly.
• Positively represent Triangle to customers, family members and community organizations.
• Participate in biweekly team meetings and monthly supervision with the Program Coordinator.
• Other duties as assigned.
