Job summary

U.S. Department of Veterans Affairs is seeking a Community Program Manager/Anticoagulation Clinical Pharmacist Practitioner to lead the Care in the Community program, standardize processes, and coordinate with local and VISN teams to ensure safe, compliant dispensing.

The role includes collaboration with the Anticoagulation Service Program Manager, data collection, quality assurance activities, and education of students and residents.

Qualifications

• Must have a PharmD or BS Pharmacy and state licensure.
• Eligible for licensure in appropriate state and maintain active registration.
• Foreign-educated graduates must have FPGEC certification and TOEFL iBT if applicable.

Responsibilities

• Manage Care in the Community (CITC) program operations.
• Oversee CITC prescription eligibility and nonformulary requests.
• Coordinate anticoagulation service work with ASPM.
• Lead training for pharmacy students and residents.
• Ensure compliance with VA and regulatory standards.
