Job summary

The U.S. Department of Veterans Affairs seeks an Anticoagulation Program Manager/Formulary Manager CPS/CPP to provide comprehensive medication management under an anticoagulation scope of practice as defined by the medical center and VA policies and guidelines.

The role requires advanced clinical expertise, program leadership, and collaboration with physicians, pharmacists, and nurses to optimize anticoagulation outcomes and ensure safe, effective therapies across patient care settings.

Qualifications

• Pharmacy degree from ACPE-accredited program (Pharm.D. or BS Pharmacy).
• US pharmacist license active and unrestricted.
• English proficiency required for clinical practice.
• PGY1 residency preferred or equivalent experience.

Responsibilities

• Develop and manage anticoagulation and formulary programs.
• Review and document drug requests and interventions in CPRS.
• Provide clinical education to patients and staff and coordinate with care teams.
• Lead quality improvement initiatives within the pharmacy service.
