Job summary

Qualifications

• Pharmacist duties include prescription intake, verification and patient counseling.
• Knowledge of drugs, indications, dosing, side effects, and proper administration.
• Ability to communicate technical information to non-technical personnel.

Responsibilities

• Comply with agency policies and procedures as well as program specific procedures and protocols.
• Standard pharmacist duties to include prescription intake, verification and patient counseling.
• Knowledge of drugs and their indications, contraindications, dosing, side effects, and proper administration.
• Advise physicians and other health practitioners on selection, dosages, interactions, and side effects of medications.
• Evaluate appropriateness of prescription orders with consideration for patient safety and legal requirements.
• Performs drug utilization reviews (DURs) and documents interventions.
• Prior authorization management with pre-approval process for filling prescribed medication orders to ensure proper insurance coverage.
• Monitors the health and progress of patients in response to drug therapy.
• Advise patients and answer questions about prescription drugs, OTCs, and lifestyle factors.
