Job summary

People-Encouraging-People in Milford Mill, MD seeks a Full Time Professional to provide outreach to patients and coordinate health services within the ACT program. You will be a full member of the team, carrying out rehabilitation and support services and providing crisis intervention as needed.

The role requires a Certified Peer Recovery Specialist (CPRS) and strong communication, problem solving and organizational skills.

Qualifications

• CPRS certification required.
• 1-6 months on-the-job training (OJT) required for effective job performance.
• Excellent communication, problem solving and organizational skills.
• Must have a valid driver’s license and a clean driving record; ability to work independently with minimal supervision.

Responsibilities

• Conduct visits to agencies, shelters, and hospitals to engage clients for the ACT program.
• Screen clients on admission to the ACT program and obtain health history.
• Coordinate client care with community resources and refer to facilities as needed.
• Participate in ACT team meetings and clinical programs.
• Provide rehabilitation services in coordination with the team.
