Qualifications

Responsibilities

• Establish and nurture partnerships with community resources and coalitions.
• Coordinate services within the early childhood system of care and CABs.
• Identify resource gaps and maintain updated provider lists.
• Provide case consultation education to nurse home visitors about community resources.
• Market the program to referral sources and community groups.
