Job summary

The Well is seeking a Community Engagement Specialist to coordinate ConnectWell activities in Madison and Platte Counties. You will build strong relationships with community organizations, healthcare providers, schools, and public safety partners, and support outreach, listening sessions, and resource alignment.

You will help implement the Connection Prescription Network, connect partners to standardized referrals, and assist with community assessments and reporting.

Qualifications

• Experience in community engagement, behavioral health, outreach, coalition-building, or related fields preferred.
• Experience working with community organizations, healthcare providers, schools, public safety, nonprofit partners preferred.
• Experience in rural communities and with diverse populations preferred.

Responsibilities

• Build and maintain relationships with community organizations, behavioral health providers, healthcare partners, schools, public safety agencies, and other stakeholders.
• Recruit organizations to participate in the Community Partnership Network and support engagement/retention.
• Coordinate outreach and listening sessions, develop accessible bilingual outreach materials, and participate in community events.
• Support implementation of the Connection Prescription Network and connect partners to referral processes and resources.
• Maintain current information on community resources, assist with assessments, surveys, and reporting.
• Collaborate with leadership and staff; travel frequently within the assigned county and to Madison/Platte Counties.
