Job summary

A community support provider is seeking part-time staff in Monroeville for direct client care. Responsibilities include supporting individuals in daily activities and community engagement, documenting interactions, and facilitating connections. Job Duties

100% Community Participation Supports are generally responsible for giving direct client care and programming to individuals in community on a 1:1 or 1:2-3 staffing ratio, participating in the development and implementation of the Individual Service Plans, ensuring that each individual participates in social/leisure/recreational activities based on their choices and preferences, providing social, emotional, and environmental supports to ensure the happiness, health, and safety of the participants in order to provide them with skills needed to meet their goals regarding employment. This job includes transporting and participating in community activities especially volunteer work with the individual and assisting them in completing tasks assigned to them during that time. For some opportunities, extra training or extra clearances may be required. You will be required to transport individuals in your personal vehicle.

  • Provide active and engaged supports that integrate with a person’s capacities and preferences
  • Teach individuals the skills required to be a self-advocate and properly communicate their preference in how and when they receive supports
  • Actively support individuals with daily living activities including but not limited to personal cleanliness, toileting, personal safety, mobility, daily management skills including but not limited to maintaining proper nutrition, home management, budgeting, medical related activities, appropriate communication, recognizing and dealing with emergencies, exercising their rights, completing tasks assigned to them and developing relationships members of the community as needed.
  • Proactively deal with conflict in a tactful manner and teach those skills to individuals supported.
  • Support individuals to develop and explore opportunities in their community based off their personal preference, interests and choices focusing on activities that provide them with skills needed for potential future employment through volunteer work. Assist to identify said opportunities and contact to arrange participation in said opportunities.
  • Facilitate new and ongoing connections with community members and volunteer opportunities.
  • Teach, demonstrate, and work with community members to ensure full community inclusion for individuals with disabilities.
  • Complete all necessary documentation required daily. Provide quality content in notes, including choices, interaction, supports given, assistance required, responses, connections, and activities occurring during program day for the individuals supported that day.
  • Complete all annual trainings required.
Requirements
  • At least 18 years of age.
  • High school diploma or equivalent
  • Driver’s license with good driving record
  • Personal vehicle and proof of insurance
  • Must be able to pass criminal clearances (Act 34 criminal record check or FBI clearance if applicable). Some volunteer opportunities may require Act 33 clearances as well.
