Qualifications

• Current Paramedic certification required; Community Paramedicine certification preferred.
• Minimum of 3 years of experience as a paramedic, at least 1 year in a community healthcare setting preferred.
• Experience in chronic disease management, patient education, and case management.

Responsibilities

• Deliver direct patient services including evaluations and case management.
• Conduct in-home health assessments and administer treatments.
• Collaborate with community resources to coordinate care.
