Limelight Health is seeking a Community Paramedic in Cincinnati, Ohio to provide mobile integrated healthcare supporting high-risk patients. This role focuses on patient assessments, chronic disease management, and community outreach to enhance patient care.
The ideal candidate will be certified as a Paramedic, have experience in community healthcare, and demonstrate strong skills in patient education and case management. A structured program emphasizes clinical quality and healthcare collaboration.
IHCare+ is a mobile integrated healthcare company that partners with health systems, payors, and community agencies to keep high‑risk patients safe at home. Our paramedics deliver patient‑centered, in‑home care that blends EMS expertise with care management, chronic disease support, and social needs navigation. IHCare+ focuses on Mobile Integrated Health (MIH) and community paramedicine services in and around Ohio, supporting patients after hospital or ED visits and those with complex, chronic conditions. We work as an extension of patients’ care teams, closing gaps between clinic visits through proactive home visits, telehealth‑supported care, and targeted interventions that reduce unnecessary hospital visits and avoidable readmissions. Our teams operate in a highly collaborative environment, working alongside nurses, advanced practice providers, care coordinators, and social service partners to deliver the right care, in the right setting, at the right time, with the right provider.
IHCare+ paramedics provide scheduled, non‑emergent home visits focused on assessment, early intervention, and patient coaching rather than traditional transport‑driven EMS response. Typical activities include head‑to‑toe assessments, medication review, vitals and diagnostic checks, chronic disease monitoring, and escalation or teleconsultation with partnering clinicians when needed. Our Community Paramedics also help identify and address barriers like transportation, health literacy, and access to primary care or behavioral health services, connecting patients with community resources and support programs. Documentation is completed in an electronic health record, supporting data‑driven quality improvement and program outcomes tracking.
