Med-Care Ambulance provides emergency medical services for 11 communities in Northern Oxford County, Maine. Our EMS providers perform 911 emergency response, inter-facility transport and community paramedicine services within the rural communities. Med-Care Ambulance is currently hiring a full-time Paramedic or an AEMT seeking paramedic licensure for a hybrid role providing in our Community Paramedicine program.
Position Overview: In this hybrid role, you will deliver Community Paramedicine services—such as post-discharge follow-ups, chronic disease support, fall-risk screening, medication reconciliation, and health education—while also maintaining strong 911/transport skills with regularly scheduled ambulance shifts.
Key Responsibilities
- Conduct scheduled patient visits in homes and community settings to assess needs, obtain vitals, and provide protocol-driven care under medical direction.
- Support care transitions (e.g., post-ED or post-hospital follow-up), including symptom check-ins, medication reconciliation, and identifying barriers to recovery.
- Provide patient education and coaching related to chronic disease management (e.g., CHF/COPD, hypertension, diabetes), wellness, and prevention.
- Perform fall-risk screening and home safety assessments; connect patients with appropriate resources and referrals.
- Collaborate with physicians, hospitals, primary care, behavioral health, public health, and social service partners to coordinate care and reduce avoidable 911/ED utilization.
- Document assessments and interventions in the designated electronic system and complete required program reports/metrics.
- Participate in quality improvement activities, case reviews, and program development as needed.

- Location: Facility located at 290 Highland Terrace, Mexico, ME, and at locations within the surrounding communities.
- Typical hours: Monday-Friday between 0800 to 1600 with some flexibility as the community paramedicine program demands.
Required Qualifications
- Maine Paramedic licensure or AEMT level providers willing to seek advanced certification.
- Current BLS/CPR certification; ACLS required. PALS and/or PHTLS/ITLS are strongly preferred.
- Valid driver’s license with an insurable driving record.
- Ability to work both independently (community paramedicine) and as part of an ambulance crew.
- Strong patient communication skills and comfort providing care in non-traditional settings.
- Proficiency with electronic documentation and basic computer skills.
Preferred Qualifications
- Community Paramedicine licensure.
- Prior experience in community paramedicine / MIH, care coordination, case management, or public health outreach.
- Experience working with high-utilizer patients, complex chronic conditions, behavioral health, or substance use support services.
- EMS field training officer (FTO) experience or interest in mentoring/education.
- EMS instructor certification.
System Information
- Three ambulances staffed daily, two 24/7 and a third based on system demand.
- 48-hour work week with built in paid overtime for full-time employees (24-hour shifts on a 1 on 2 off, 1 on 4 off rotation)
