Job summary

BJC Independence Center in St. Louis is seeking a Community Outreach Case Worker to connect adults with mental health services and community resources, including housing and food assistance, through daily field work.

You will use motivational interviewing, document services clearly, re-engage clients after missed appointments, and mentor colleagues. Requires a Bachelor's Degree in Human Services or related and a valid Class D (IL) or Class E (MO) driver's license.

Qualifications

• Bachelor's Degree in Human Services or related.
• No prior experience required.
• Valid Class D (IL) or Class E (MO) driver's license (or obtain within two weeks of start).

Responsibilities

• Provides community support services to complex clients to include clients on care plan, using treatment plan interventions that result in positive outcomes.
• Completes timely documentation of services that clearly describe the need for the service, the intervention provided, the relationship to the treatment plan, the provider of the service, the date, the actual time and setting of the service, and the individual's response to the service.
• Contacting individuals and/or referral sources following missed appointments in order to re-engage and promote recovery/resiliency efforts.
• Provides mentorship and job shadowing to community support colleagues.
