Job summary

Qualifications

• Two years of post-secondary education in Human Services or related field plus three years relevant experience.
• Preferred: experience with community outreach, case management or resource coordination.
• Bonus: bilingual abilities and partnerships with local organizations.

Responsibilities

• Meet with individuals and families to learn needs and connect with programs and resources.
• Explain available services and agency programs in a clear, helpful way.
• Partner with local organizations to expand support for participants.
• Complete applications, maintain client files, and ensure accurate documentation.
• Help participants navigate challenges to reach their goals.
