GENERAL DESCRIPTION

The Community Outreach Manager is responsible for the day-to-day management of special projects’ outreach operations in support of the mission of New Horizon Health Center (NHHC). Under the direction of the Director of Community Services and Grant Management, this position provides operational leadership to the Community Health Worker (CHW) team by supervising daily outreach activities, implementing approved outreach initiatives, managing community engagement efforts, and ensuring the successful execution of departmental goals and objectives.

The Community Outreach Manager is responsible for supervising Community Health Workers, managing outreach operations, monitoring productivity, coordinating community events and educational presentations, and developing relationships with businesses, employers, schools, healthcare providers, and community organizations that increase awareness of NHHC services, improve access to healthcare, and support organizational growth through new patient opportunities

The Community Outreach Manager is responsible for ensuring outreach activities are conducted efficiently, staff performance expectations are met, outreach resources are effectively utilized, and departmental objectives established by the Director are successfully implemented. The position continuously evaluates outreach operations and provides recommendations to improve efficiency, strengthen community engagement, and expand access to NHHC services.

The Community Outreach Manager exercises independent judgment in managing the daily operations of Special Projects outreach activities while referring matters involving departmental strategy, budgets, grant administration, staffing, major partnerships, organizational initiatives, and new program development to the Director of Community Services and Grant Management for review and approval.

Due to the nature of community outreach activities and community events, it may be necessary for the employee to work extended hours, evenings, weekends, or variations of the usual schedule. Flexible hours may be assigned to ensure adequate community representation and the successful implementation of outreach initiatives.

Primary responsibilities

• Manages the day-to-day operations of Special Projects outreach activities to ensure the successful implementation of approved outreach initiatives that support NHHC's mission, departmental goals, and organizational objectives.
• Manages community outreach activities, including health fairs, educational presentations, screenings, outreach events, wellness initiatives, and approved special projects.
• Develops and maintains relationships with businesses, employers, schools, healthcare providers, community organizations, faith-based organizations, and other partners to increase awareness of NHHC services and identify opportunities for outreach, education, referrals, and new patient growth.
• Implement outreach activities associated with assigned grants and special projects in accordance with approved work plans and timelines.
• Maintains accurate outreach records and prepares required reports, ensuring timely and accurate documentation.
• Works collaboratively with clinical departments and marketing to implement approved outreach campaigns and promote NHHC services.
• Monitors outreach productivity, presentations, referrals, community engagement, business development activities, and new patient opportunities; recommends operational improvements to the Director.
• Represents NHHC at community meetings, business meetings, health fairs, and educational presentations.
• Participate in Quality Improvement, Performance Improvement, required training, and departmental meetings.
• Performs assigned duties in accordance with established NHHC policies, procedures, and applicable regulations. Exercises independent judgment in managing day-to-day outreach operations while referring significant personnel, financial, strategic, compliance, partnership, grant-related, and programmatic matters to the Director of Community Services and Grant Management for review and approval.
• Performs other duties as assigned.

OTHER DUTIES:

• Verbal ability is required to express effectively ideas and views when speaking to groups, center directors, personnel and in community outreach.

Education

Bachelor's degree in Public Health, Health Administration, Business Admini stration, Sociology, Communications, Social Work, or a related field is required. Master's degree preferred. Equivalent combinations of education and directly related experience may be considered in accordance with NHHC policy.

Experience

Minimum of two (2) years of supervisory experience managing staff, coordinating outreach activities, or leading community-based programs.
