Job summary

Aspire Health and Community Services in Braintree, MA is seeking an Outreach Case Worker to provide community-based services for adults with chronic behavioral health issues. You will work with a multidisciplinary team to assess needs, advocate for access to resources, and coordinate care with families and guardians.

The role requires a valid driver’s license, CPR/First Aid certification, and MAP certifications.

Qualifications

• HS Diploma and a BA degree in Psychology or related field preferred.
• One or more years of experience in the Mental Health field.
• Working knowledge of community-based treatment and regulations is preferred.

Responsibilities

• Provide community outreach for a large caseload of adults with chronic behavioral health issues.
• Advocate for clients to ensure quality of care and access to services.
• Conduct assessments and make treatment recommendations.
• Serve as liaison with families and guardians; coordinate with multidisciplinary team.
• May provide transportation and assist with medications; document in EHR.
