Job summary

Aspire Health and Community Services is seeking an Outreach Case Worker to serve adults in the community with chronic behavioral health issues. The role involves working with a multidisciplinary team to connect clients with services and supports in the Braintree area.

The position emphasizes outreach, documentation, and coordination of care, with requirements including a valid driver's license and CPR/First Aid certification. Full-time schedule Tue–Sat, with day and evening shifts.

Qualifications

• HS Diploma or higher required; BA in Psychology or related field preferred.
• 1+ years of experience in the Mental Health field strongly preferred.
• Working knowledge of community-based treatment, funding sources, and treatment regulations is preferred.

Responsibilities

• Provide community outreach and be responsible for a large caseload of clients.
• Advocate for clients ensuring quality of care; recommend resources and access to services.
• Make assessments; address client needs and recommend treatment.
• Serve as primary liaison with family and guardians; collaborate with a multidisciplinary team.
• May provide transportation; administer medication; manage client finances.
• Provide documentation; update and maintain client health records at Braintree site using an electronic health record system.
