Aspire Health and Community Services is seeking an Outreach Caseworker to support adults in the community with chronic behavioral health issues from our Braintree site. This full-time role involves a schedule with Sunday-Thursday shifts, including a mix of 9a-5p and 11a-7p weeks.
The Caseworker collaborates with a multidisciplinary team including clinicians, nurses, and peers to coordinate care, advocate for clients, and ensure access to needed services.

Schedule: Sunday -Thursday Hours are a Combination of 9a-5p (3 days) and (2) 11a-7p shifts weekly
Outreach Caseworker is a full-time outreach position providing services to adults residing in the community, experiencing chronic behavioral health issues (diagnoses may include substance use disorder, schizophrenia, dual diagnosis, schizoaffective disorder, physical issues, etc.).
The Caseworker works with a multidisciplinary team that includes Master level clinicians, nursing staff, peer specialists, and others involved in client care.
Compensation is based on candidate experience, qualifications, and certifications and licenses. Annual ranges listed are for a full-time, 40 hours per week position.
Salary Range: $22.40 - $25.25 USD
