Vinfen is seeking a dedicated case management professional to contribute to high-quality services for individuals in need. The role includes direct case management, training in daily living skills, and crisis intervention support.

Candidates should have experience in human services, a high school diploma or higher, and be prepared to work flexible hours including overnights and weekends as required.

Formación

• At least one year in human services or related field preferred.
• Bachelor’s degree in counseling psychology, human services, or related; experience may substitute for training.

Responsabilidades

• Implement service plan interventions with persons served.
• Provide direct training to develop daily living skills (symptom, medication, money management, cooking, cleaning, hygiene, social skills).
• Maintain and update documentation in program records.
• Advocate for persons served.
• Communicate with coworkers, supervisors and providers.
• Escort clients to community using transportation as needed.
• Monitor status and provide crisis intervention as needed.
• Report incidents to on-call manager/supervisor.
• Perform housecleaning tasks as assigned.
• Attend in-service trainings and staff meetings.
• Provide coverage for the program including overnights, days, evenings, weekends and holidays.
• Adhere to program/company policies and regulations.
• Store, administer, document, and dispose of medication per MAC and policy.
