Job summary

Seacoast Mental Health Center, Inc. is seeking an Outreach Specialist/Case Manager in Portsmouth, NH.

The role focuses on coordinating services, providing direct support, advocacy and assertive outreach to adults with mental illness within the Adult Services Department. Ideal candidates hold a Bachelor's degree in Social Work, Psychology, or Human Services (or equivalent) and have at least one year of field experience.

Qualifications

• Bachelor's degree in Social Work, Psychology, Human Services or equivalent required.
• One year of field experience preferred.
• Excellent communication skills, both written and verbal required.
• Valid driver's license with clean driving record.
• CPR/First Aid training recommended.

Responsibilities

• Coordinate services and provide direct support, advocacy and assertive outreach to adults with mental illness.
• Join the Portsmouth team in the Adult Services Department and contribute to community-based care.

Key Responsibilities

The Adult Services Department of SMHC is looking for an energetic, dedicated, experienced individual to join our Portsmouth team. This position requires coordinating services and providing direct support, advocacy and assertive outreach to adults with mental illness. The ideal applicant must possess a Bachelor's degree in Social Work, Psychology, Human Services or the equivalent and preferably have one year of experience working in the field.

Other Requirements

• Must also demonstrate excellent communication skills, both verbally and in writing.
• The Outreach Specialist must hold a valid driver’s license and be able to demonstrate a "clean" driving record.
• CPR/First Aid training is recommended.

Salary -$47,014.50
