Job summary

Diamond Healthcare in Pennsylvania is seeking a proactive Community Liaison to grow census by building strong relationships with hospitals, case managers, physicians, families, and community partners. This outward-facing role spends most of its time in the field rather than at a desk.

You will visit local hospitals to coordinate discharges and referrals, meet families during the admissions process, represent the facility at events, and work with the admissions team to ensure smooth transitions

Qualifications

• Bachelor's degree in marketing, healthcare administration, or related field preferred.
• Prior experience in healthcare marketing, sales, or community outreach strongly preferred.
• Excellent interpersonal and communication skills.
• Willingness to travel within the community.
• Flexible schedule including evenings or weekends for events.
• Valid driver's license and reliable transportation.

Responsibilities

• Visit local hospitals regularly to build and maintain relationships with discharge planners, case managers, and social workers.
• Serve as the primary point of contact for hospital case managers coordinating patient discharges and referrals to the facility.
• Meet with families to answer questions and guide them through the admissions process with warmth.
• Represent the facility at community events, health fairs, and outreach opportunities.
• Develop and maintain relationships with physicians, senior communities, and referral sources.
• Track referral activity and follow up on leads and inquiries.
• Coordinate facility tours for prospective residents and their families.
• Distribute marketing materials and represent the facility's brand in the community.
• Collaborate with the admissions and administrative team to ensure smooth transitions for new residents.
• Report on outreach activity and market feedback to leadership.
• Stay informed on local competitors and market conditions to identify growth opportunities.
