Responsibilities

• Manage referrals into the organisation, assessing and completing initial assessments, managing risk and priority.
• Collaborate with individuals to co-produce meaningful goals rooted in participation, wellbeing, and confidence.
• Provide 1:1 interventions and use physical activity as an assessment/intervention tool.
• Maintain and review caseloads with regular outcomes and updates to plans.
• Liaise with GP surgeries, Primary Care Networks and external agencies to build partnerships.
• Deliver person-centred practice and promote participation in meaningful occupations.
• Complete required records and data, and participate in audits and CPD opportunities.
