Job summary

UniversaCare Inc. in New Haven and Valley, Connecticut, seeks a compassionate Occupational Therapist (OT) to provide high-quality therapy to clients across community settings.

This role offers clinical independence, meaningful patient relationships, flexible scheduling, and opportunities for a diverse caseload in home health and outpatient contexts. You will evaluate, treat, and educate patients and families, document findings, and collaborate with a multi-disciplinary team.

Qualifications

• Current Connecticut OT license.
• Graduate of an accredited Occupational Therapy program.
• Current CPR/BLS certification.
• Ability to work independently with a flexible schedule.
• Experience in home health, community-based, outpatient, or geriatric care is a plus.

Responsibilities

• Evaluate patients' functional abilities and develop individualized treatment plans.
• Provide occupational therapy services based on each patient's clinical needs and goals.
• Help patients improve independence with activities of daily living, mobility, safety, and functional skills.
• Monitor patient progress and adjust treatment plans as appropriate.
• Educate patients, families, and caregivers on therapeutic techniques and home programs.
• Collaborate with physicians, nurses, care managers, and other healthcare professionals.
• Complete accurate and timely clinical documentation.
• Maintain compliance with applicable federal, state, and organizational requirements.
• Travel throughout the assigned New Haven and Valley service area as needed.
