Job summary

Signature Health Inc. is seeking a Supervisor, Community Nursing to lead the nursing team, ensure timely access to services, and oversee clinical pathways in ambulatory and inpatient settings.

You will provide direct patient care as needed while championing high-quality, patient-centered care and collaborative care coordination. You will supervise day-to-day operations, coach staff, and participate in recruitment and performance reviews, with opportunities to influence care protocols and program

Qualifications

• Bachelor Degree in Nursing preferred.
• Valid unencumbered RN license in the state of Ohio required.
• 3+ years of experience in an ambulatory, acute, residential or inpatient psychiatry preferred.
• 2+ years of nursing supervisory experience in healthcare preferred.
• American Heart Association (AHA) Basic Life Support (BLS) certification required.
• Excellent written and verbal communication skills.
• Proficiency in Microsoft Office products.

Responsibilities

• Supervise daily objectives of the Community Nursing team including access to services, assessments, census reconciliation, staff productivity, communications, and documentation.
• Support cross-department collaboration and coordination of community-based patients; foster positive relationships with external partners.
• Oversee team performance, conduct regular coaching, and participate in annual performance reviews.
• Plan and maintain daily nursing assignments, assess suicide risk, support medication access/adherence, and oversee handoffs between nursing and providers.
• Provide ongoing communication, teaching, and training for all team members.
• Lead team meetings and drive process improvements to enhance productivity, quality, and efficiency of the team.
• Assist Nursing Director in developing evidence-based clinical protocols and care coordination metrics; monitor program effectiveness.
• Ensure clinical activities comply with state/federal standards and evidence-based practices.
• Participate in recruitment and retention activities.
• Assist team with patient care as needed, plan prescriber pairings, and participate in after-hours admissions.
