Merakey seeks a Mental Health Professional in Lafayette, LA to join an Assertive Community Treatment Team. The MHP will deliver individual and group therapy, develop person-centered treatment plans, and conduct psychosocial assessments for clients with severe mental illness.

Qualifications

• Must have an active driver's license within the state of Louisiana.
• Willing to work in a community based setting.
• Must have an active PLPC/LMSW within the state of Louisiana.

Responsibilities

• Provide individual therapy and group therapy to consumers.
• Create and review person-centered treatment plans and crisis plans.
• Complete psychosocial assessments and maintain timely clinical documentation.
• Provide crisis intervention and support.
