Tri-County Behavioral Healthcare is seeking an Adult Mental Health Specialist to deliver psychosocial rehabilitation, case management and skills training to adults with chronic mental illness. The role serves Montgomery County in home and community settings from a central office location.

You will engage clients, coordinate with community providers, and document in the electronic health record, while adhering to trauma-informed care principles.

Qualifikationen

• QMHP credentialing requirement under Texas TAC rules (Rule §353.1415).
• Bachelor’s degree in Behavioral Science or related discipline is required.
• Valid driver’s license and insurability through Tri-County.

Aufgaben

• Provide psychosocial rehabilitation skills training and community-based mental health case management.
• Support goals identified in each person-centered recovery plan.
• Locate, engage, and meet with individuals on your caseload; coordinate with providers and resources.
• Complete documentation within required timeframes in the electronic health record.
