Goodwill of the Heartland in Cedar Rapids, IA seeks a Supported Community Living Associate to build relationships with participants, identify needs, and connect them with community resources.

This full-time role involves monitoring treatment plans, documenting progress, managing expenses, and providing transportation as needed. A high school diploma or GED is required and a reliable vehicle is preferred. Hourly wage is $16.78.

Qualifications

• High school diploma or GED required.
• Six months experience in training and/or rehabilitation required. Post-secondary education may be substituted for experience requirement.
• Reliable vehicle, valid Iowa driver's license, proof of auto-insurance, and successful completion of a motor vehicle record screening, dependent and adult child abuse, and criminal background check

Responsibilities

• Develop and maintain effective relationships with clients.
• Identify and maintain current information about community resources that may benefit program participants.
• Identify treatment and service needs of each individual served.
• Monitor and evaluate progress of the treatment plan, assisting and facilitating as appropriate.
• Monitor and control expenses and ensure efficiency in rehabilitation services, including billable hours and client funding.
• Write quality case notes and assist SCL Specialist with ISP Development and Progress Reports.
• Provide transportation to participants as needed.
• Have access to a safe, reliable vehicle.
