Job summary

Achieva in Pittsburgh, PA is seeking a Community Living Supervisor to coordinate the daily operation of Community Home(s) and support individuals in living a life of personal significance. You will work with families and staff, chart outcomes, and serve as on-call contact on a rotating schedule.

Responsibilities include developing a weekly staff schedule, maintaining records, administering medications (certified), and guiding teams to meet individual support plans.

Qualifications

• Preferred Bachelor's degree, or equivalent with some previous experience.
• Ability to complete one-person physical transfers.
• Valid Pennsylvania driver’s license.
• Must meet Act 33/34 clearance requirements.
• Must obtain CPR certification within 6 months of the start date.
• Must pass Medical Administration Training (Provided by Achieva).

Responsibilities

• Provide direct care supports to people with intellectual or developmental disabilities in daily living, social, and recreational activities.
• Ensure compliance with policies; maintain in-house and personal records; assist with upkeep of the home.
• Implement formal and informal outcome plans to help the person reach personal goals.
• Develop and adjust the weekly staff schedule to cover all shifts.
• Communicate concerns with the Community Homes Supervisor and participate in staff meetings.
• Oversee Annual Assessments and supporting documents; implement identified outcomes.
• Distribute funds, obtain receipts, and assist with banking as advised.
• Administer medications (upon completion of certification; training provided).
• Perform other duties as assigned.
