Job Summary

The Community Living Supervisor is responsible for overseeing the daily operations of assigned Community Homes, ensuring the health, safety, and well-being of individuals with intellectual or developmental disabilities. This role involves providing direct care, supervising staff, and implementing outcome plans to help individuals achieve their personal goals. The supervisor will also ensure compliance with all regulatory and organizational policies.

  • Enroll in the Front Line Supervisor (FLS) certification program
  • Receive a pay rate increase ($1/hr) and one-time bonus upon completion

Key Responsibilities

  • Provide direct care support to people with intellectual or developmental disabilities and specialized care in daily living, social, and recreational activities.
  • Ensure compliance with the Department's Policies and Procedures relative to Community Homes.
  • Assemble, maintain, and monitor all in-house and personal records.
  • Assist with and ensure the upkeep and maintenance of the home.
  • Implement formal and informal outcome plans to assist the person in achieving their personal outcomes.
  • Develop the weekly staff schedule and adjust staff schedules due to vacation, absenteeism, or illness with prior approval. Ensure all shifts are covered.
  • Communicate and resolve program and individual concerns with the Community Homes Supervisor on a regular basis. Participate in general staff meetings.
  • Assign and ensure completion of each person's Annual Assessment and supporting documents, and ensure the implementation of the outcomes identified in the Individual Support Plan.
  • Distribute individual funds, acquire and submit receipts, and assist with banking as advised.
  • Coordinate medications (upon completion of certification; all necessary training provided).
  • Career Development and Advancement Opportunities, in partnership with National Alliance of Direct Support Professionals (NADSP). Acquire Front Line Supervisor (FLS) badge/certification within 1 year of enrollment, coincide pay rate increase and bonus incentive.
  • Acquire CPR certification within six months of the start date.
  • Ensure all prescribed medication is dispensed and detailed per P&P in partnership with the individual.

Qualifications

  • Equivalent to a Bachelor's Degree from an accredited college or university and/or 2 plus years of experience working directly with individuals with an intellectual disability is preferred.
  • An accredited certification as a Nursing Assistant is highly preferred.
  • Ability to complete one-person transfers is essential.
  • Must meet Act 33 and 34 Requirements.
  • Access to transportation and a valid driver's license are necessary.
  • Supervisory experience is preferred.
  • Must obtain First Aid/CPR certification within two months of the start date.
  • Successful completion of Medication Administration training, provided by Achieva.

Qualifications:

The equivalent of a Bachelors Degree from an accredited college or university and 2 years of experience working directly with individuals with an intellectual disability is preferred.

Good interpersonal and organizational skills as well as strong written and verbal communication skills are needed to be successful in this role. The ability to complete one person transfers is essential. Must meet Act 33 and 34 Requirements. Reliable transportation is needed for success in this role; and a valid drivers license is highly preferred. Supervisory experience is preferred in this position.

In addition to paying for federal and state clearances, Achieva offers 24 hours of training, annually, relevant to Human Services. In addition, we have implemented Medication Administration training, which would need to be completed successfully. This training assists our Community Living Professionals in ensuring all prescribed medication is dispensed and documented per P&P in partnership with the individual.
