Job summary

Reach for Resources, Inc. is seeking a Community Living Specialist to support individuals with disabilities in their homes and in the community around Hennepin County, with occasional visits to our Edina office.

Travel and mileage reimbursement are provided in this role, which offers a flexible, community-based schedule. The position emphasizes independent living skills coaching, person-centered support, and collaboration with families and teams to develop, implement, and evaluate a tailored

Qualifications

• Experience in disability services, social services, or related field preferred.
• Strong communication skills, both verbal and written.
• Ability to develop and implement independent living skills.
• Comfortable with community-based travel.
• Valid driver's license and reliable vehicle.

Responsibilities

• Promote independence in life skills through teaching, coaching, and modeling.
• Develop and implement Community Living Plans with individuals and teams.
• Travel to support individuals in their homes and community within the service area.
• Maintain courteous, professional representation of the agency.
• Collaborate with families and care teams to monitor progress.
