Job summary

Placement Partners is seeking an Independent Living Specialist (Skills Trainer/Direct Support Professional) to help adults with developmental disabilities live independently in the SW Metro area (Jordan, Bloomington, Eagan, Long Lake).

You will provide individualized support, assist with daily living, promote healthy lifestyles, and advocate for clients with community resources. This role offers a flexible schedule and opportunities to grow with a dedicated team.

Qualifications

• Must be at least 18 years old with a high school diploma or GED.
• Must have a valid driver's license, reliable vehicle, and current auto insurance.
• Experience with individuals with developmental disabilities is preferred but not required.
• Willingness to meet clients in the community and drive own vehicle.

Responsibilities

• Provide individualized support to consumers to develop and maintain independent living skills.
• Assist consumers with ADLs, healthy lifestyle, community engagement, and accessing resources.
• Support consumers in achieving personal goals through problem-solving and communication.
• accompany consumers on community outings and advocate with service providers.
• Develop and monitor consumer goals, maintain documentation, and communicate with the team.
• Participate in staff meetings, ensure compliance with regulations, maintain weekly schedules via Google Calendar.

Position Available:

Independent Living Specialist (Skills Trainer/Direct Support Professional)

• Provide individualized support to consumers in developing, maintaining, and improving the skills necessary to live independently and successfully within the community.
• Assist consumers with activities of daily living, healthy lifestyle habits, community engagement, and accessing community resources.
• Support consumers in achieving personal goals by demonstrating creative problem solving, interpersonal skills, and healthy decision making.
• Accompany consumers on community outings and serve as an advocate with service providers, while promoting independence, respect, and self-confidence.
• Develop, implement, and monitor individual consumer goals, objectives, and outcomes while maintaining communication with interdisciplinary team members and completing required reports and documentation.
• Participate in staff meetings and training, ensure compliance with Minnesota State regulations and company policies, and maintain weekly activity schedules through Google Calendar reporting.

Qualifications

• Must be at least 18 years old, have a high school diploma/GED, a valid driver’s license with a reliable vehicle and proof of up to date auto insurance.
• Must maintain an acceptable motor vehicle record.
• Prior experience working with individuals with developmental disabilities is preferred, but not required.
• Frequently required to meet with consumers in the community and drive personal vehicle.

Job Type: Full-time or Part-time

Pay: $21.50 - $22.50 per hour

Expected hours: 20 - 35 per week, depending upon selected candidate's preference
