Job summary

Caliber Therapies is seeking a Life Skills & Community Support Professional to work in home and community settings, promoting independence and meaningful participation for clients across ages. This is a non-clinical role focused on coaching, mentoring, and hands-on support to help individuals reach person-centered goals.

Responsibilities include building relationships, teaching living skills, facilitating community participation, and documenting progress in line with funding requirements.

Qualifications

• Must have strong communication skills (verbal and written).
• Ability to work independently in home and community settings.
• Basic computer proficiency and ability to complete electronic documentation.

Responsibilities

• Build positive, professional relationships with clients and their support systems.
• Assist individuals in developing and practicing independent living skills.
• Support participation in community outings and recreational activities.
• Teach and reinforce communication, problem-solving, decision-making, and social skills.
• Assist clients in identifying and achieving person-centered goals.
• Encourage healthy lifestyle habits, physical activity, and emotional wellness.
• Support participation in volunteer opportunities, educational activities, employment readiness, and community involvement when appropriate.
• Promote self-advocacy, confidence, and independence while providing the least restrictive support necessary.
• Maintain accurate and timely documentation according to company and funding source requirements.
• Communicate professionally with caregivers, case managers, and the Caliber Therapies team.
• Follow all company policies regarding safety, confidentiality, professionalism, and mandatory reporting.
