Job summary

Trinity Health Senior Communities is seeking a Part-time Community Life Aide in Indiana to support planning, implementation, and evaluation of activity programs for residents. As a Community Life Aide, you will interview residents and families to set relevant goals, document progress, and assist with transportation and participation in events.

Qualifications

• High School Diploma or equivalent.
• Strong customer service and communication skills.
• Ability to adapt to changing environment and handle fast-paced workload.
• Ability to work with residents of varying cognitive abilities.

Responsibilities

• Assist in planning, implementing, and evaluating community life programs per resident care plans.
• Interviews with residents/families to set meaningful goals.
• Maintain and document program activities in Care Tracker.
• Coordinate transportation and collaborate with other departments.
• Participate in training and required in-service sessions.
• Support discharge planning and ongoing activity care plans.
