The Community Liaison interfaces with hospitals, skilled nursing homes, board and care homes and outpatient discharge planning services, and physician offices. Provides and acts as a community resource and public relations representative in facilitating safe quality healthcare in our communities. The Community Liaison is expected to generate appropriate patient referrals/ admissions by establishing and maintaining professional relationships with all referral sources.
