Role Description

This is a full-time, on-site role for a Community Liaison Hospice located in Oakbrook Terrace, IL. The Community Liaison Hospice will focus on building and maintaining relationships with community partners, educating the community and healthcare providers about hospice and palliative care services, and coordinating care for patients and families. The role involves extensive interaction with various stakeholders, ensuring patient referrals, conducting outreach activities, and supporting the hospice's mission to deliver compassionate care.
