Join to apply for the Community Liaison/Health Care Navigator role at Volunteers of America-Greater New York.

Volunteers of America-Greater New York (VOA-Greater New York) is an anti-poverty organization working to end homelessness in the New York area by 2050. We operate over 60 programs across New York City, Northern New Jersey, and Westchester County, including emergency, transitional, and permanent housing. Our services extend to families experiencing homelessness, survivors of domestic violence, people living with HIV/AIDS, veterans, youth aging out of foster care, adults with behavioral health or substance use needs, and others in need, including a special education preschool program.

We are seeking new talent for the position of Community Liaison/Healthcare Navigator. This role works closely with Veterans, their primary care providers, and the interdisciplinary treatment team to ensure access to healthcare and benefits. The Navigator assists Veterans in navigating complex healthcare systems, accompanying them to appointments and helping with benefit applications.

Through the Supportive Services for Veteran Families (SSVF), a community-based outreach program, we identify low-income veteran households and connect them with supportive services to promote housing stability and prevent homelessness. Learn more about the program at https://www.voa-gny.org/ssvf.

Minimum Qualifications:

  • Associate degree, or 60 credits in Human Services or related field, or High School Diploma or GED with 1 year of experience in healthcare or social services.
  • Knowledge of the VA medical system preferred.
  • Excellent communication skills, both written and verbal, and basic computer skills.

Principal Responsibilities:

  • Connecting Veterans to VA health care benefits or community health services.
  • Accompanying Veterans to appointments at VA medical centers and other institutions.
  • Providing case management, health education, and care coordination.
  • Collaborating with healthcare providers and community leaders.
  • Serving as a resource for education and support, developing relationships, and reviewing care plans.
  • Acting as a liaison between the SSVF program and VA or community clinics.
  • Supporting outreach efforts to identify eligible Veterans.
