Role Description The Community Liaison is a full-time hybrid role based in the San Antonio, Texas Metropolitan Area, with a mix of community-based work and some work-from-home flexibility. This role involves building and maintaining relationships with referral sources, community organizations, and healthcare providers to increase awareness of hospice and home care services. Day-to-day tasks include conducting outreach visits, participating in community events, providing education about hospice care, and responding promptly to inquiries from families and partners. The Community Liaison collaborates with clinical and administrative teams to coordinate patient referrals, ensure smooth transitions into care, and support a positive experience for patients and families. This position also requires maintaining accurate records of outreach activities, preparing reports, and using communication tools and technology to stay connected with internal and external stakeholders.

Qualifications

• Strong community engagement and community outreach skills, with experience building partnerships and representing an organization in public settings.
• Excellent communication abilities, including clear verbal communication, active listening, and professional written correspondence.
