Job summary

Wellspring Family Services in Morgantown, WV, seeks a Community Liaison to assess, plan, implement, monitor, and evaluate services addressing client needs. You will provide supportive counseling, case management, and behavior management tailored to each treatment plan, including CSEDW services for youth and families.

  • Experience with at-risk youth/families and trauma-focused treatment.
  • Must have valid driver’s license and ability to travel with own vehicle.

Responsibilities

  • Provide supportive counseling, targeted case management, behavior management and crisis intervention as prescribed in the treatment plans.
  • Coordinate with Clinical Supervisor and treatment team.
  • Complete CANS and other assessments as indicated by the team.
  • Develop, implement and review treatment plans in accordance with state and agency requirements.
  • Meet weekly service hours as determined by supervisor.
  • Assist in implementing new service delivery programs developed by state agencies.
  • Engage with youth and families and communicate effectively with all team members.
  • Assess and respond to crisis situations.
  • Collaborate with community agencies to access supports for youth and families.
  • Advocate for youth and families by attending MDTs and IEPs.
  • Maintain confidentiality and HIPAA compliance; complete incident reports as needed.
  • Utilize required technology to perform job efficiently.
  • Maintain current certifications for core trainings (e.g., CPR, CPI, First Aid).
  • Coordinate services including assessment, intensive case management, in-home therapy, day treatment, and school-based behavioral health.

Job description

As a Community Liaison, your responsibilities will include assessing, planning, implementing, monitoring, and evaluating the services necessary to address a client's needs. You will offer supportive counseling services that encompass daily management and problem-solving assistance, along with behavior management interventions tailored to each client's individualized treatment plan. You will also assist in the implementation of Children with Serious Emotional Disorder Waiver, CSEDW, Services for the youth and families identified and referred to Wellspring Family Services. You will collaborate and coordinate with the youth and the family, service providers, and support network to develop an individualized plan of care driven by the perspectives of the family and youth.

Qualifications:
  • Bachelor’s Degree in Social Work, Psychology, or related behavioral health services field
  • Minimum of one (1) year experience or equivalent school/community service experience with at-risk youth/families, knowledge of and experience in trauma- focused treatment and child development
  • Ability to travel with own vehicle and posses a valid driver’s license and insurance statement
Responsibilities:
For traditional Wellspring Family Services clients:
  • Provide supportive counseling, targeted case management, behavior management (BMI) and crisis intervention as prescribed in the treatment plans.
  • Coordinate, in collaboration with the Clinical Supervisor and other treatment team members
  • Completes the CANS and other screeners or assessments as indicated by the treatment team
  • Assist with the development, implementation and review of every treatment plan and any other service plans in accordance with state and agency requirements
  • Meet weekly services hours as determined by supervisor.
  • Assist in the implementation of new service delivery systems/programs developed by DHHR or other corresponding state agencies.
  • Engage and communicate effectively with youth and their families/legal guardians, and all team members.
  • Assess and respond effectively to crisis situations.
  • Collaborate with team members to access the needed supports and services for each youth and family served by working with youth and family-serving agencies and organizations in the community.
  • Advocate for youth and their families by attending appropriate meetings, such as MDT’s and IEP’s
  • Adhere to confidentiality, HIPAA, mandated reporter, and risk management policies and procedures including but not limited to completing Incident Reports.
  • Utilize technology provided in order to perform job efficiently and effectively
  • Must possess current certifications for all core trainings (ex. CPR, CPI, and First Aid)
For traditional CSEDW clients:
  • Provide coordination of services that will include but are not limited to assessment and evaluation, intensive case management/In-home family support, In-home supportive therapy, day treatment, behavior management, intensive home-based mental health services, school-based behavioral health services, substance abuse, intensive outpatient services, crisis services,mobile crisis response,youth coaching, peer support, respite services, therapeutic mentoring, and therapeutic foster care
  • Work to encourage a shift in family’s reliance on professional supports to natural supports, to enable families to build long-term connections and supports within their community.
  • Coordinate all wraparound plan meetings by scheduling the meeting and notifying all team members in a timely manner.
  • Develop service plans that are individualized and tailored for each family. These plans will encourage innovation, creativity, and appropriate goal setting based on the individual family.
  • Maintain the appropriate caseload determined by the state of WV, while meeting the weekly minimum service hours set forth by Crittenton Services/Wellspring Family Services.
  • Assist to enhance cross system collaboration and assure early linkage/intervention
  • Provide proper documentation and timely completion of all paperwork associated with the requirements of the position.
  • Establish a positive working relationship with all members of the team. Work cooperatively with all staff for the good of the agency/youth and family, and project a positive public image.
  • Coordinate and attend all required meetings and necessary case reviews.

Salary: $17 to $20 per hour (based upon licensure and experience)
