Position Overview:
As theCommunity Outreach Liaison, you will serve as a key connector between Silverline Home Health and the broader healthcare and senior services community. Your role will focus on building relationships with referral sources, educating the public and professionals about our services, and ultimately helping more patients access the care they need at home.
Key Responsibilities:
Cultivate and maintain strong relationships with physicians, hospital discharge planners, case managers, senior living communities, and other referral sources
Promote Silverline’s mission and services through educational events, presentations, and targeted outreach campaigns
Identify new community and provider partnerships to grow referral base and expand brand visibility
Track and report referral trends, outreach activities, and marketing efforts to meet defined goals
Collaborate with clinical and administrative teams to ensure a smooth referral and intake process
Represent Silverline at networking events, health fairs, and community gatherings with professionalism and warmth
Maintain up-to-date knowledge of Medicare regulations and home health eligibility guidelines to provide accurate information to partners
Qualifications:
2+ years of experience in healthcare marketing, community outreach, or business development (home health or senior care preferred)
Strong communication and interpersonal skills, with a natural ability to connect with diverse audiences
Self-motivated, goal-oriented, and able to work both independently and collaboratively
Solid understanding of healthcare systems and referral processes
Reliable transportation and valid driver’s license required
