Job summary

Healthcare California Home Health and Hospice is seeking a Community Liaison to expand relationships with hospitals, physicians, and community organizations in Fresno and surrounding areas. This full-time role emphasizes outbound outreach, education on services, and collaboration with clinical teams.

The candidate will educate providers and families, coordinate with referral sources, and participate in community events to promote agency services and patient access.

The Details

We areseeking a Community Liaison Representative to join an extraordinary team!We are expanding our operations,and we are looking for collaborative team members to join us. Our ideal candidate is an outgoing, experienced, team-oriented medical marketing/sales professional who is driven to work. This candidate must have excellent phone/text/email communication with the team and be someone who is driven toward quality outcomes and teamwork.

Responsibilities and Duties:

The Community Liaison's goal is to work collaboratively to establish strategic relationships with community hospitals, nursing homes, physicians, and community organizations.

The Community Liaison has the following responsibilities:

• Develop and maintain relationships with physicians, hospitals, skilled nursing facilities, assisted living communities, case managers, discharge planners, and other referral sources.
  
• Educate referral sources on agency services, admission criteria, and patient eligibility.
• Represent the organization at community events, health fairs, networking functions, and professional associations.
• Assist referral sources, patients, and families in understanding available services.
• Work with the clinical field team and management to develop a strategic marketing plan.

Qualifications and Skills:

• Associate's or Bachelor's degree in Marketing, Business, Healthcare Administration, Communications, or a related field preferred.
• Minimum of 1-3 years of experience in marketing/sales in pharmaceutical, assisted living facilities, skilled nursing facilities, or other healthcare sales experience considered.
• Excellent interpersonal and relationship-building skills.
• Ability to educate healthcare professionals, patients, and families regarding services and eligibility requirements.
• Strong follow-up and account management skills.
• Must have valid driver's license, reliable transportation, and current automobile insurance.
• Ability to pass background screening and meet agency requirements.
