Job summary

Credent Care LLC is seeking an Integrated Community Support Professional to promote independence and community participation for program participants. The role involves assisting with transportation, healthcare access, and social engagement in a supportive home and community setting.

You will work with a team to develop individualized plans, monitor progress, and provide hands-on training in communication and self-sufficiency. A valid driver’s license and reliable transportation are required.

Qualifications

• High school diploma or equivalent required; additional education or certifications are a plus.
• Ability to provide person-centered care with strong communication and teamwork.
• Must have a valid driver’s license and reliable transportation.

Responsibilities

• Foster a safe, supportive environment that promotes independence and community integration.
• Enhance community engagement and socialization for participants.
• Identify transportation needs and coordinate access to services.
• Coordinate with healthcare providers and schedule appointments as needed.
• Train in effective communication, active listening, and conflict resolution.
